( view more ) these axes in 25 patients with IVNC (10 asymptomatic-LV ejection fraction [LVEF] > or = 50%; 15 symptomatic-LVEF < 50%) and 15 healthy controls. In all cases, the systolic velocity pattern featured 2 distinct peaks (SW1, SW2). These peak velocities and the intervals from the electrocardiographic Q wave to each peak (Q-SW1, Q-SW2) were recorded for each axis, and group means were calculated. The asymptomatic group displayed significantly higher long axis SW2 and significantly longer long axis Q-SW1 than the controls. The symptomatic group had significantly lower SW1 and SW2 on both axes and significantly longer short axis Q-SW1 than the asymptomatic group and the controls. Long axis Q-SW1 and short axis Q-SW1 in the symptomatic group were significantly longer than the corresponding control findings, but were not significantly different from the corresponding asymptomatic group findings. CONCLUSION: In conclusion, patients with IVNC exhibit distinct systolic myocardial shortening velocities along the long and short axes of the LV. We aimed to evaluate the value of admission MPV and WBC-C for the prediction of IRA patency, in patients with acute STEMI treated with primary percutaneous coronary intervention. METHODS: Blood samples were obtained on admission in 351 STEMI patients. The patients who had thrombolysis in myocardial infarction (TIMI) 3 flow in initial angiography constituted the IRA patent group and others having less than TIMI 3 flow constituted the IRA occluded group. RESULTS: In 16% of the patients, IRAs were found to be patent on initial angiography. Patients in the IRA occluded group had higher admission MPVs (9.3+/-1.2 vs. 8.6+/-1.3 fl, P<0.001) and higher WBC-C (13.3+/-4.8 vs. 11.0+/-2.9, P=0.002) compared with patients in the patent IRA group. In regression analysis, WBC-Cs [beta, 0.131; odds ratio (OR), 1.140; 95% confidence interval (CI), 1.043-1.245, P=0.004)] and MPV (beta, 0.519; OR, 1.680; 95% CI, 1.206-2.339, P=0.002) were found to be independent predictors of occluded IRA. The best cutoff value of MPV for predicting an occluded IRA was determined to be 8.55 fl with a sensitivity of 74% and a specificity of 60%. CONCLUSION: MPV and WBC-C at admission might be valuable in the prediction of IRA patency and in planning the need for adjunctive therapy to improve outcomes in patients with STEMI undergoing percutaneous coronary intervention. METHODS: We assessed the echocardiographic features of hydatid cysts in 16 patients with CE (9 women and 7 men; mean age, 41 +/- 18.3 years), all of whom underwent cardiac surgery. The proposed classification system identifies 3 types of CE lesions: active (unilocular or multilocular and echolucent, showing double-layered cyst wall and hydatid sand); transitional (shrunken as a result of reduced intracystic pressure, and showing water lily sign); and inactive (completely degenerated contents creating the ball-of-wool sign). Preoperative echocardiographic findings (lesion location, imaging appearance [unilocular/multilocular, solid/semisolid], echocardiographic classification/type, number of lesions) were compared with computed tomographic, intraoperative echocardiographic, surgical, and parasitological findings. RESULTS: Preoperative echocardiography revealed 18 hydatid cysts (10 myocardial, 7 pericardial, 1 on the ascending aorta). In all, 10 lesions appeared multilocular, 6 unilocular, and 2 solid. A total of 11 were active, 5 transitional, and 2 inactive. Computed tomography identified 20 lesions total, therefore, two were missed on echocardiography. The preoperative echocardiographic findings correlated well with intraoperative echocardiographic, surgical, and parasitological findings. CONCLUSION: The 3 types of CE lesions defined in this proposed classification system feature distinct echocardiographic characteristics. This new system is reliable and practical, and could assist with diagnosis and rapid treatment of CE. ( view more )ix patients with MS and sinus rhythm (study group, 20 female, mean age 33 +/- 8 years) and a well-matched control group composed of 21 healthy volunteers (15 female, mean age 35 +/- 6 years) were enrolled in the study. In each patient left atrial (LA) thrombus was excluded by transesophageal echocardiography. sP-selectin levels were compared between study patients and controls, and between peripheral and LA blood. Changes in sP-selectin level 24 h and 4 weeks after PMBV were also analyzed. RESULTS: A significantly higher basal sP-selectin level was noted in study group. After PMBV, the mitral valve area (MVA) increased and the transmitral mean gradient decreased significantly. At 24th hour after PMBV, sP-selectin level decreased from 1080.38 +/- 143.87 ng/ml to 960.00 +/- 103.26 ng/ml (p < 0.0001) and at 4th-week follow-up it was decreased from 960.00 +/- 103.26 ng/ml to 879.61 +/- 98.16 ng/ml (p < 0.0001). No significant difference was found between peripheral and LA blood regarding the sP-selectin level. Correlation analysis between the difference in MVA measured 24-hour after and before PMBV and the difference in sP-selectin level measured 24-hour after and before PMBV showed a significant direct relationship between these variables. CONCLUSION: This study suggests that in patients with MS and sinus rhythm sP-selectin level increases probably due to turbulent flow across the stenotic valve. After PMBV sP-selectin shows a progessive decline, which is directly correlated with the increase in MVA. ( view more ) normal RV systolic function, RV diastolic functions of asymptomatic patients with mitral stenosis of moderate or severe degree were compared to symptomatic ones by pulsed-wave tissue Doppler echocardiography in this study. MATERIALS AND METHODS: The study included 40 patients with mitral stenosis. Patients were classified into two groups according to New York Heart Association (NYHA) class (asymptomatic group NYHA class I, symptomatic group NYHA class II-III). RV diastolic functions were evaluated via pulsed-wave tissue Doppler in terms of isovolumic contraction time (IVCT), systolic wave velocity (S), isovolumic relaxation time (IVRT), early diastolic wave velocity (E'), and late diastolic wave velocity (A'). RESULTS: Statistical difference was not determined in systolic parameters (S, IVCT) between the two groups. However, IVRT (54.21 +/- 11.93 msec vs 86.19 +/- 16.23 msec, P < 0.0001) and A' wave amplitude (10.16 +/- 2.14 cm/sec vs 13.55 +/- 3.35 cm/sec, P < 0.0001) were higher whereas E' wave amplitude (11.68 +/- 1.60 cm/sec and 10.25 +/- 2.73 cm/sec, P = 0.009) and E'/A' ratio (1.18 +/- 0.23 vs 0.76 +/- 0.17, P < 0.0001) were lower in group 2. CONCLUSION: In isolated mitral stenosis, pulsed-wave tissue Doppler may be used for the detection of RV diastolic pathology. Diastolic functions of RV may deteriorate in the presence of normal systolic functions in symptomatic patients with isolated mitral stenosis. ( view more ) patients (44 male, mean age 41 +/- 18 years) with the diagnosis of IVNC were evaluated in this retrospective cohort. Its prevalence was found to be .14%. Forty-seven patients (70%) had class I/II functional capacity. Fifty-seven patients (85%) had electrocardiographic abnormalities, and the most common one was left ventricular (LV) hypertrophy (25%). LV systolic function was depressed in 44 patients (66%), with a median ejection fraction (EF) of 35% (range: 20%-48%) at diagnosis. Multiple regression analysis revealed that age at initial presentation, the total number of affected segments, and the ratio of non-compaction/compaction (NC/C) were the independent predictors of LV systolic dysfunction. Familial occurrence of IVNC was 33%. During a mean follow-up of 30 months (range: 9-50 months), major complications including ventricular tachycardia, heart failure requiring hospitalization, and cerebrovascular events were observed in 36%, 34%, and 9% of the patients, respectively. Ten patients (15%) with IVNC died in this study. LVEF at initial presentation and functional capacity at last visit were found to be independent predictors of mortality. CONCLUSION: This study suggests that IVNC is a form of cardiomyopathy with higher prevalence and relatively better prognosis than previously reported. Age at initial presentation, ratio of NC/C, and number of affected segments seem to be major determinants of LV systolic dysfunction, while initial LVEF and last functional capacity predict mortality in this cohort. ( view more ) of the long axis (LA) and short axis (SA) of the LV as determined from pulsed Doppler tissue imaging in two groups of patients with similar degrees of LV dysfunction and similar LV sphericity: patients with IVNC and patients with idiopathic dilated cardiomyopathy (DCMP). METHODS: Pulsed Doppler tissue imaging was used to record systolic myocardial velocities along the LA and SA of the LV in a group with IVNC (n = 13) and a group with DCMP (n = 13). The groups were matched for mean extent of LV dysfunction and mean sphericity index. In all 26 patients, the systolic myocardial velocity pattern for each axis featured two distinct peaks (SW1 and SW2). These peak velocities and the intervals from the electrocardiographic Q wave to each peak (Q-SW1, Q-SW2) were recorded for each axis, and group means were calculated and statistically compared. RESULTS: There were no significant differences between the IVNC and DCMP groups with respect to any of the pulsed Doppler tissue imaging variables. Concerning the data for SW1 and SW2, intragroup comparisons revealed no significant differences in either group. In each of the two groups, LA-Q-SW1 was significantly shorter than LA-Q-SW2, SA-Q-SW1, and SA-Q-SW2. Each group's LA-Q-SW2 and SA-Q-SW2 values were also statistically similar. CONCLUSION: The results indicate that, given comparable degrees of spheric remodeling and comparable LV dysfunction, patients with IVNC and patients with DCMP show similar contraction properties along the LA and SA of the LV. ( view more )e of the hemodynamic changes in transmitral gradient (TG) in response to Valsalva maneuver and functional status, systolic pulmonary artery pressure, and left atrial function in patients with MS in sinus rhythm. METHODS: The study prospectively evaluated 42 patients (37 female, mean age 40 +/- 10 years) with a diagnosis of pure MS in sinus rhythm. Patients were divided into 3 groups according to their New York Heart Association (NYHA) functional class. In all, 12 patients were in NYHA functional class I, 17 patients in class II, and 13 patients in class III. TG, including the mitral peak, mean, and late filling gradients, were calculated by the modified Bernoulli equation at rest and during the strain phase of Valsalva maneuver. The percentage changes in all TG in response to Valsalva were calculated. Traditional echocardiographic parameters for atrial function were also measured. RESULTS: We found that, in patients who were more symptomatic according to NYHA classification, a reduction in TG in response to Valsalva maneuver was blunted and associated with higher systolic pulmonary artery pressure compared with those who were asymptomatic or had mild symptoms. Patients in NYHA class III decreased mitral mean pressure gradient by only 13.5%, significantly less (P < .001) than those in classes I (53.9%) and II (41.7%). However, despite the fact that left atrial ejection fraction was low in all patients, it was not associated with functional status in patients with MS. CONCLUSION: This study, for the first time, suggests that echocardiographically determined changes in TG in response to the Valsalva maneuver may provide an objective, noninvasive means of evaluating patients with MS. The rates of hypertension and levels of serum creatinine, cholesterol, and triglycerides were statistically higher in group I compared with the other 2 groups. Patients with diabetes and LBBB (group I) had significantly higher scores for the severity (Gensini score) of CAD (p <0.001) and more 3-vessel disease (p <0.001). After adjustment for hypertension, hypertriglyceridemia, and hypercholesterolemia with covariance analysis, the presence of LBBB was also associated with a higher Gensini score in patients with diabetes compared with those with diabetes but without LBBB and those with isolated LBBB (p <0.001). METHODS: Four hundred and thirty-six patients (300 females) were reviewed retrospectively. The patients were classified according to RBBB existence in electrocardiography: group A included 83 patients with RBBB existence, and group B contained 353 patients without RBBB. The patients were further classified as subjects who had an echocardiographic valve score < or =8 (325 patients, group 1) and those with a valve score >8 (111 patients, group 2). RESULTS: The mean age of the patients was similar in groups A and B. In group A, the mean mitral valve gradient was higher (12.63 +/- 4.43 vs. 10.58 +/- 3.37 mm Hg; p < 0.0001), the mitral valve area smaller (1.05 +/- 0.2 vs. 1.14 +/- 0.52 cm2; p = 0.011), and the systolic pulmonary artery pressure higher (53.5 +/- 16.2 vs. 46.9 +/- 13.2 mm Hg; p = 0.001) than in group B. The mean age and mitral valve area were similar in groups 1 and 2. The mean mitral valve gradient (10.5 +/- 3.7 vs. 12.3 +/- 3.0 mm Hg; p < 0.0001) and systolic pulmonary artery pressure were higher in group 2 (46.7 +/- 13.3 vs. 52.5 +/- 15 mm Hg; p < 0.0001). At the end of first growing period, 30 seedlings from each treatment were harvested and measured for height (SH), root collar diameter (RCD), root dry weight (RDW), stem dry weight (SDW) and total dry weight (TSDW). These parameters varied significantly among treatments and were lower in zeolite added media. SH negatively correlated with Na and K content and C/N ratio of growing media but positively correlated with Mn content of media. SDW and TSDW had positive correlation with N, Fe, Mn, total porosity and loss of ignition, and had negative correlation with pH, Ca, Na and K content. SETTING: Tertiary care referral centers. PATIENTS: Fifteen adult patients (mean age, 21 +/- 6 years; age range, 9 to 30 years) undergoing successful total correction at a mean age of 12 +/- 5 (patients) and 15 healthy, matched volunteers (control subjects). RESULTS: There was evidence for a slight right ventricular diastolic dysfunction in the patients. Mean FVC (88 +/- 9% vs 109 +/- 12% predicted, respectively) and FEV1 (89 +/- 9% vs 109 +/- 12% predicted, respectively), although being within the normal range, were also decreased in comparison to those of control subjects (p < 0.0001). Maximal oxygen consumption (V(O2max) decreased in both groups (55 +/- 16% vs 61 +/- 23% predicted, respectively; p = 0.5); however, there were more individuals with severely decreased values among the patients (p = 0.05). V(O2) at the anaerobic threshold was also decreased in patients (33 +/- 15% vs 51 +/- 8% predicted, respectively; p = 0.004). The maximum tolerable exercise time was 17.3 +/- 4.5 min in patients vs 21.2 +/- 6.4 min in control subjects (p = 0.06). CONCLUSIONS: The exercise capacity after delayed repair was good in general compared to matched control subjects; however, exercise capacity may be slightly limited by ventilatory dysfunction, low anaerobic threshold, and lack of physical fitness despite New York Heart Association class improvement after undergoing the operation. In Phase-II, they were instructed to apply a pea-size amount (PS) and finally they were demonstrated to use the transverse technique (TT) in Phase-III. Statistical analysis was obtained by using Anova and t-test. As a result, most of the children (82 patient) preferred to use ST rather than TT (18 patient). A decrease in the amount was obtained with both of the shaped orifices after the instruction of PS or demonstration of TT (p< 0.05).
